1. Scope of Work By signing this Contract below I (Client) authorize representatives of Lice Charmers LLC (LC) to perform head lice treatment for myself and all persons as described in '3. Guarantee Terms.' 2. Payment LC will provide head lice treatment for the Client. A travel charge, one treatment or one hour minimum may apply. Full payment is due upon presentation of the invoice. 3. Guarantee Terms — READ THIS SECTION By signing this contract the Client understands and agrees to the following terms: 3.A. That all persons residing or working within the household and all immediate family, family-in-law, hired persons, and any persons in regular contact with the household must be cleared of lice or complete a full lice treatment from LC to qualify for the guarantee. Everyone not seen at the initial appointment must be checked or treated by LC within 24 hours of it, and in no case later than the end of the following day. Anyone seen after that does not qualify, and the guarantee is void. 3.B. If any person, as described in section 3.A., is concealed or misrepresented, the guarantee is void and unenforceable. 3.C. If any LC employee or representative feels unsafe or is threatened and ceases providing service, the guarantee is void and unenforceable. 3.D. If any of the above terms are not met, this Guarantee and all published, advertised, or otherwise stated LC guarantees are void and unenforceable. 4. Guarantee — READ THIS SECTION If the Client satisfies all Guarantee Terms and received a qualifying, full treatment from LC for an active lice infestation and provides physical evidence of an active lice infestation within 60 days of the initial treatment, LC will provide checks and treatment at one of our clinics only for persons who received a check or treatment at the initial appointment. All checks and treatments under guarantee must be completed within 24 hours of the initial appointment. Clients who only received a check or checks at their initial appointment and did not have an active lice infestation do not qualify for free checks or treatments. 5. Missed Appointment and Cancellation Policy The Client agrees to provide a minimum of 24 hours' notice for any appointment cancellation or rescheduling. If notice is not provided at least 24 hours prior to the scheduled appointment time, or if the Client is not present at the location within 30 minutes of the scheduled start time, a missed appointment fee will be charged. For mobile appointments: the missed appointment fee shall be the full minimum service charge. For clinic appointments: the missed appointment fee shall be $75. By signing this agreement, the Client authorizes Lice Charmers LLC to charge the payment method on file or invoice the Client for this fee. The Client consents that LC can provide their services and communicate with me via mobile phone, messages, e-mail and any kind of online communications, provided that these communications comply with privacy regulations. The Client agrees that this contract may be electronically signed and that the electronic signatures in this contract are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility. If you choose to opt-out, a paper contract will be provided at your appointment.
